All-on-4 Implant Treatment

All-on-4 (also known as Teeth-in-a-Day) is a full-arch implant technique that uses four implants per arch to support a fixed prosthesis. In suitable cases, a provisional fixed prosthesis may be fitted in the same session.

 

When compared with separate implants, this approach uses fewer implants than some other full-arch options; whether it is appropriate, and how it compares with alternatives, depends on individual clinical assessment.

All-On-4 Dental Implants, also known as pro-arch, teeth-in-a-day or full mouth implants, is a method where 4 or 6 implant fixtures are placed into each jaw at one go.

 

Within the same day, a bridge prosthesis will be fitted over each fixture, linking up with your jawbone to support a fixed prosthesis as part of a multi-stage treatment plan.

  • All-On-4 may be suitable for patients looking to replace multiple missing teeth in an arch, or for those with teeth that the dentist has assessed as having a poor prognosis. Suitability is determined on a case-by-case basis after clinical assessment.

 

Patients who consider All-On-4 dental implants often share some of the following:

 

  1. Loss of several teeth in an arch
  2. Bone loss in the jaw
  3. Terminal dentition (where teeth have a poor prognosis and require replacement)

Stage 1: Consultation

  • Detailed Exploration of Concerns

When you attend your consultation at Nuffield Dental, our dentist will discuss your concerns regarding the procedure and assess whether All-On-4 may be suitable for you. 

 

  • Full Dental Assessment

The dentist will examine your general health and the condition of your teeth and gums. This may include X-rays and CBCT (Cone-beam computed tomography) scans to obtain detailed 3D images of your jaw. These steps help determine suitability for the All-On-4 procedure or other treatment options.

 

  • Individualised Treatment Plan

The dentist will explain your condition, the benefits and risks of each option, and prepare a treatment plan in discussion with you.



Stage 2: All-On-4 Procedure

  • Anaesthesia

Before the procedure begins, the relevant areas are numbed with local anaesthetic. Most patients report only pressure or vibration sensations during the surgery; let your dentist know if you experience any discomfort.

  • All-On-4 Dental Implant Procedure

The dentist will first carry out any necessary extractions, then place the All-On-4 implant fixtures into your jaw bone.

 

  • Interim All-On-4 Dental Implant Bridge Prostheses

Our in-house laboratory prepares your interim All-On-4 dental implant bridge prostheses. In suitable cases these are fitted to your implant fixtures in the same sitting; the interim prosthesis is later replaced by a final restoration after healing.


Stage 3: Post-operative

The body requires a few days to begin accepting the implants. A follow-up appointment is scheduled so the dentist can assess the healing process.

  • A bridge will be fitted onto the implants once healing is complete.
  • If the standard All-On-4 plan is unsuitable due to jawbone loss, the dentist may discuss a Zygomatic Implant as an alternative. This option may be considered for patients with severe bone loss in the upper jaw who are still candidates for implants following clinical assessment.

  • Fewer implants per arch. Using four implants per arch (in suitable cases) reduces the number of fixtures compared with some other full-arch protocols. Whether this is appropriate depends on individual clinical assessment.

  • Anaesthesia associated risks.
    As with all surgeries, this is highly dependent on the health related risks that come along with an individual. This would mean any pre-existing conditions that would be declared upon the first consultation with Nuffield dentists. However, anaesthesia problems are deemed rare and usually can be avoided with a transparent discussion with your dentist.
  • Implant failure. This happens when your body rejects the implant, which also means that your jaw bone is unable to fuse with the All-On-4 implant.

 

Risk of infection. Any surgery carries a risk of infection, which can be reduced with proper aftercare. The specific infection to look out for is peri-implantitis. Symptoms of peri-implantitis include:
  1. Bleeding or pus around the implant area.
  2. A fever.
  3. Throbbing pain.
  4. Redness and swelling in the gums.
  5. Bad breath.
  6. Difficulty chewing.
  7. The implant feels loose.

Here at Nuffield, our panel of providers usually make a follow-up appointment at the 1-week mark to check if the bridge has been cleaned properly. This is followed by the 2 week mark where the bridge and stitches are removed. At the 8 week mark is where the healing begins and if everything is healthy. Finally at the 6-month mark is where the final bridge is made. 

Some of the side effects that patients have experienced after All-On-4 procedure are:

 

  • Discomfort or swelling where the implants are located.
  • Bleeding at the implant area.
  • Difficulty adapting to bridge bulk. This may affect speech for certain individuals temporarily. 
  • Sore throat.
  • Jaw discomfort or a stiff jaw.

These side effects usually last for a week or two, and these side effects can be alleviated with medication issued by your dentist.
